In DDO, Damage Reduction (DR) is a measure of how much damage your character can absorb before you start losing hitpoints. The value of the DR is subtracted from each hit you take. For example, you have a DR 5 and you get hit for 4 points of damage.
This means that you don't have to worry about your hitpoints going down because your DR absorbed all the 4 damage and you still have 1 extra. On the other hand, if you have DR 5 but you were hit by 10 points of damage, then your hitpoints will be down by 5 points. DR applies only to physical weapon damage.
According to DDO Wiki:
Player characters (but not monsters, apparently) can gain "Active DR" when they are holding down shift to block. Active DR is displayed as "blocking DR" in your inventory sheet. Passive DR is visible on the character sheet, or as buffs in your buff bar (stoneskin). Multiple DR entries will be listed, and only the highest one which is not bypassed by a particular attack's properties (like Adamantine) applies.
So, Active DR is the damage reduction you gain if you go on blocking stance, but there are other sources of DR. Here are some of them:

If you work on your DR, you will surely be hard to take down, especially if you are a melee type. If you are a caster, you can be less squishy than the others of the same class. It's worth getting items that help increase your DR. Of course, you can also change armors or items that give DR depending on the quests you do. Say, if you are doing a quest where it is highly infested with enemies using slashing weapons, then you will need to use armor or items that have DR/slashing.
It's important to know what your enemies will be using against you, so you can easily adjust and equip the right armor or items. Some players don' think DR is very important but it spells the difference between dying fast and staying alive.
